CHADIS is Here

We have launched a system called CHADIS, which helps us provide your family with the best care possible.
CHADIS is a secure, HIPAA compliant site that allows you to answer many of the developmental questions that we ask during a well-child visit. Based on your child’s age, CHADIS will send you a welcome email to create an account if there are any questionnaires that can be completed prior to your appointment.
