Looking for school and camp registration forms? Can’t remember your child’s future appointment date? Our team at North Atlanta Pediatrics is very excited to be utilizing our patient portal, Follow My Health, to offer you resources and answers to these questions and more!
In order to access your child’s health information through the portal, you will need to be invited by our practice to become an Authorized Proxy User for your child’s account. Please give our office a call, or complete one of our portal authorization forms, in order to be sent an invitation email.

Once you have completed the registration process as a proxy user you will notice that the information displayed is that of the authorized individual. Simply click “hello, [your name]” in the top righthand corner to be given a dropdown box with your child’s name. Click on the name of the child whose information you wish to view to access their account. If you are accessing the portal through the mobile platform, click the “accounts” button to populate the dropdown box.
Follow My Health makes it easy to have all family members connected through one portal account. If you are a patient of another practice that utilizes the Follow My Health platform, you can use your current login information when connecting from the authorized proxy user invitation.
Here are some of the main features of the patient portal!
- Update demographic information and pharmacy preferences.
- Print or email a PDF copy of uploaded camp/school registration forms (please allow 24-72 business hours after forms request is submitted to the office via email or phone request).
- View account balances, sign up for e-statements, and pay your statement balance online.
- Send secure non-urgent messages to your provider (please allow 24-48 business hours for a response).
- Request medication refills through secure portal messages (new features coming soon).
- View your child’s vital signs, immunizations, medications, and measurements from all past appointments.
